Guide to Acne scars
One of the most common questions asked in dermatology offices among patients with acne is how to get rid of acne scars. This is a important issue because, as acne has its own problems, sometimes the resulting acne scar can be as cosmetically disturbing as the original acne. Many advances are been done to facilitate the appearance of acne scars, but not over-the-counter remedies are currently available. The following discussion and suggestions should be carefully reviewed with your experienced dermatologist.
The first step in the treatment of acne scarring treat the underlying acne. Without an effective anti-acne regimen acne scar treatment will be vain, as more scar will occur despite treatments. For a more thorough discussion about acne and acne regimens, we refer to these items in DermBlue Learning Center or accompanying EzineArticles:

Your regimen may include simple over-the-counter acne care or prescription topical like Retin-A or Tazorac ® ®. Other treatments include antibiotics, Blu-Light Photodynamic therapy and oral treatment with isotretinoin. These choices are made between you and your experienced dermatologist. Acne scars can be divided into two main groups: rolling acne scars and ice-pick scars. Rolling scars can best be described as the hills and valley are really amplified with tangential lighting. Ice-pick scars, is the crisp, deep pitting holes, as if an ice pick was stabbed into the skin. The treatment of these two different types of scarring is different in both the methods used and the degree of success. For rolling scars, which are surgical, ablative and non-ablative methods to reduce scarring. Surgical methods include subcision treatment where a needle / knife is placed under the scar and moved side to side to loosen up for the underlying scarred tissue. Another treatment option is using a filler such as Restylane ® or medical-grade silicone to fill the scar. Non-ablative therapies include using an infrared laser to heat up and remodel the tissue underlying the scar. Ablative therapies include dermabrasion (manual sanding of the skin) or ablative lasers such as Er: YAG or CO2 lasers that remove the top of the skin lasers and essentially even with "hill-tops and valleys" of the rolling scars.

For ice-pick scars, surgical techniques include punch removal, when a cookie-cutter circular instrument used to remove scars under anesthesia and a nail may or may not be able to help with healing. Another new technique involves using 90% trichloroacetic applied with a toothpick. This serves to damage the skin over the scars and promote scar remodeling. The other treatments mentioned above also apply: the use of fillers agents, dermabrasion and laser treatment.
Generally, a non-ablative laser treatment may reasonably lead to 50-70% improvement in rolling acne scars. Subcision and use of fillers is ideally used for limited rolling acne scars, punch excision treatment may be used if insulated ice-pick scars are noted. For more extensive scarring in either type of dermabrasion or ablative laser treatments tend to be the best solution.
Many of these treatments are skin-type specific and must be determined after consultation with your dermatologist. Although these treatments are not covered by insurance, can the cosmetic result is well-worth the price. Be sure to discuss all possible therapeutic options and combinations as well as the reasonably expected results.
